'Brokerage in the community' should provide information, advice, peer support and professional support with information and advice, planning and putting plans into action

This should be provided by a variety of agencies and people working together in the community

Working with other partner organisations, we want to support local people to create new ways to provide the support people need to live a life that's right for them
